STEPS:

1. Do you have a referral from your PCP /Chiropractor? This is Recommended

2. Type of insurance - Please verify our practice is in Network with your insurance.

3. If not in Network with your insurance. In network tax ID #

- Fee due at time of Consultation and will be applied to your insurance coverage / deductible / out of network benefits.

4. Your insurance will determine if your Botox will be covered for medical necessity.

5. Once your insurance covers your Botox for treatment, we will call you to set up your appointment, after we receive it in the office.

6. We submit the procedure to your insurance even if out of network. In some cases, we may need to collect payment upfront and will submit payment rendered to your insurance.

7. If you have a balance due to the practice for non-covered procedures, this will be due prior to rescheduling your next session. 

 

Botox for Migraine therapy is often every 3 Months and depends upon your Insurance Coverage.

BOTOX® is a prescription medicine that is injected to prevent headaches in adults with chronic migraine who have 15 or more days each month with headache lasting 4 or more hours each day in people 18 years or older.Medical consultation prior to ordering medication. Clients must meet specific criteria. 

**Additionally, Botox can be used to enhance lip area with what is called "a lip flip", be used for cosmetic purposes (wrinkles, etc.) and treat hyperhidrosis. 

Dysport is an injection that temporarily treats moderate to severe frown lines between the eyebrows by reducing specific muscle activity. 
 

Xeomin® , made by Merz Pharmaceuticals, is approved by the US FDA for the treatment of adults with cervical dystonia or blepharospasm and Moderate to severe frown lines between the eyebrows. Xeomin® has now been approved for some cosmetic indications – Moderate to severe frown lines between the eyebrows. Similar to other drugs in this category, including Botox® and Dysport®, that also have FDA approval for the treatment of facial wrinkles. Now that Xeomin® is available and FDA approved, doctors can use it both on-label and off-label to treat the same facial lines treated by Botox® and Dysport®, including frown lines, crow’s feet and forehead wrinkles.
